What To Expect

When you call to schedule your appointment or when you request your appointment through our website, our office manager will speak with you and obtain pertinent information about you: name, phone number, and insurance information.

We will then work with you to find the earliest available appointment that accommodates with your schedule. Once we've got you scheduled, we'll ask you to fill out the New Patient Form the day of your appointment, or download and print it from our website (complete it before your appointment and bring it with you).

Government Issued Photo ID (ex. Driver's License)
Your most recent insurance card
Your medication list
Your allergies
Completed New Patient Form

Insurance We Accept and Billing

We accept all PPO plan. For HMO plans, it requires you to have your primary care physician's name on the card. In this case, you need Dr. Judy Joseph's name on your HMO insurance card so you are covered when you visit us.
